Additional documentation needed for Name & Address File request

  1. Full Study Survey - mail or phone survey to be given to qualified participants.

  2. Study brochure or informational letter - introductory study brochure should fit into a standard business size envelop (#10), CMS does not want to inundate the beneficiary with a large amount of material.

  3. CMS beneficiary notification letter (standard-cannot be changed) - formal CMS letter notifying the beneficiary of the study and that the decision of whether to be contacted will not affect their benefits.

  4. Cost Estimate from the University of Minnesota Beneficiary Contact Service which includes overview of services to be provided, as well as the cost to provide mailing services. Additional information about the CMS Beneficiary Contact Service.


To obtain the Medicare Name & Address File, researchers will be required to show that the Medicare Names & Address File is necessary to conduct their study. In the study protocol, researchers must provide a detailed explanation of how alternative sources of information cannot be used to obtain survey participants. Alternative sources for obtaining survey participants include, but may not be limited to, the Department of Motor Vehicles driver's license lists, State voter registrations lists, and Random Digit Dialing. The relative resource intensity (e.g., money, staff time) of obtaining survey participants from other sources versus CMS is not considered a sufficient justification for use.
